Grace Compassion Home Health (Gradark Compassion Care, Inc.) is proud to be accredited by CHAP, (Community Health Accreditation Program) the independent, nonprofit, accrediting body for community-based health care organizations. It is the oldest national community-based accrediting body with more than 8,300 sites currently accredited. CHAP has the regulatory authority to survey agencies providing home health services to determine if they meet the Medicare Conditions of Participation and CMS Quality Standards. CHAP’s purpose is to define and advance the highest quality of community-based care for families in need of reliable and caring home health services.

Each one of their certified nursing and therapeutic staff members are thoroughly screened via criminal reference checks, state mandated certification checks, personality evaluations and licensure reviews before they are hired. In addition, they look for employees who are willing to go the extra mile in making sure each patient’s needs are met. They also encourage their staff to attend continuing education programs on an annual basis.

They begin with a thorough review of the physician’s report, conduct their own patient and environment assessment, and then formulate a plan of care which is evaluated monthly and update as needed to accommodate the patient’s progress.

Grace Compassion Home Health Agency specializes in helping patients stay in their homes while they receive medical treatment. They are often called upon to assist patients in their transition from a hospital or nursing home who need continuous support to prevent interruptions or setbacks in their recovery.

They have the expertise to handle a wide variety of clinical needs on both a short-term and long term basis. Their critical care certified nurses that are certified for cardiac illnesses are comfortable supporting patients with LVAD’s, home cardiac monitoring, CHF (Congestive Heart Failure), respiratory and other cardiac issues. In addition, there clinical team is experienced with tracheostomy care and wound management.

Patients recovering from, or recently diagnosed with illnesses, as well as patients recovering from procedures or surgery, often experience depression, increased anxiety, changes in behavior, and appetite. Their team understands these challenges and are prepared to offer the support necessary to help the patient and family make a successful transition, and continue with care or recovery as well as assistance with daily living activities, including household services and companion care services.
