Individual & Family Plans

FirstCare Advantage Dual SNP (HMO SNP)

A health plan, with a Medicare contract, that is designed to meet the special needs of those who are eligible for both Medicare & Medicaid—serving Carson, Crosby, Deaf Smith, Floyd, Garza, Hale, Hockley, Hutchinson, Lamb, Lubbock, Lynn, Potter, Randall, Swisher, and Terry counties.

Medicaid & CHIP

FirstCare STAR is a Medicaid program designed for low-income families, children, and pregnant women; including doctor visits, prescriptions, and more. For those not eligible for Medicaid, but who can’t afford other insurance, they offer FirstCare CHIP (Children’s Health Insurance Program)—providing health coverage for children in low-income families.
